Before participating in ANY athletic activities (preseason workouts and tryouts included), each student must have a current physical, a concussion awareness form, a player/parent release form, a consent for participation form, and a handbook acknowledgement form on file with the athletic director. Physicals remain valid for 395 days from exam date. Go HERE for all required forms.
After submitting all required forms, students must email the athletic director for a ticket, which enables them to participate. The athletic director will email students a confirmation ticket, which students will present to coaches at preseason workouts and/or tryouts. Coaches cannot accept paperwork!
