• Athletic Handbook

  Athletic Handbook (updated 6/5/25)

• Physical Form

  This is good for 395 days from the date of the exam designated by the doctor. This must be on file to try out and participate in any preseason conditioning. All physicals (Middle School and High School) must be on this document (updated 5/9/25)

• Gfeller-Waller Concussion Form

  This is good for one school year. This must be on file to try out and participate in any preseason conditioning. (updated 5/9/25)

• Consent for Participation (HS ONLY)

  This form is required by the NCHSAA prior to participating in high school athletics. (updated 7/14/25)

• Player Release Form

  This is good for one school year. This must be on file to try out and participate in any preseason conditioning. (updated 7/18/25)

• Return To Play form (post-concussion)

  This form is required of any student suffering a concussion. The RTP Protocol and RTP Form must be completed before an athlete can return to full participation (updated 5/9/25)

• NCHSAA Return to Play Injury

  This form is required of any student suffering an injury that keeps the student athlete from participating for 5 or more days. (Updated 5/9/25)

• Return to Play from Illness

  This form is required of any student suffering an illness that keeps the student athlete from participating for 5 or more days. (Updated 5/9/25)

• Signing Day Information Form

  This document is for HS students who have signed to play a sport at the college level. Please fill out all information and submit to the athletic director. (Updated 7/11/23)

• NCDOE and NCHSAA Student Eligibility Rules

  These are the rules governing eligibility status as determined by the NCDOE and NCHSAA as of 3/28/25. Additional Franklin Academy eligibility requirements related to daily attendance and grades may be found in the Athletic Handbook.

• FA Student Only Athletic Pass

  This pass is for HS Students that plan to attend games on the HS Campus. Pass is good for all Home Regular season events

• FA Family Pass

  THIS PASS IS GOOD FOR IMMEDIATE FAMILY ONLY. PASS IS VALID FOR:

  ALL HOME MS and HS SOCCER, VOLLEYBALL, AND BASKETBALL REGULAR SEASON CONTESTS ONLY.

  CONFERENCE TOURNAMENTS AND NCHSAA PLAYOFFS ARE NOT INCLUDED AND WILL INCUR A SEPARATE FEE.

• CrashCourse Concussion Video

  The NCHSAA requires this video be viewed by all coaches, parents, and player prior to participation in any athletic activities at member schools.
